- You may want to print this page to have it handy while you
prepare and submit your Code Official of the Year Nomination.
If so, select the "Print this Page" button at the bottom of this
page. But don't worry about it, there are instructions on the
page also.
- Select the link to the Interactive online form Code Official of
the Year Nomination Form at the bottom of this page:

Registration
- When the form opens, select the text box you want to enter text
into by placing your cursor on the line opposite the text box name:
Item 1 on the form is "Name Placed in Nomination." Type in the name
of your nominee. Part two of item one is "Date Nominated"
Type in the date of your nomination of this candidate.
- There is plenty of room for expanding comments on page two of
the form - just scroll down to page two. It is titled
"Additional Comments." Again, just place your cursor in the
text box and type in the information. Be sure to connect your
expanded comments to the item number on page one by entering the
item number in the text box you use to make the expanded comments.
- After completing the entries on page one, you may still wish to
add additional comments. These may also be entered in
Additional Comments.
- After completing the form you may want to print out a
copy. If so, select the "Print Form" button in the lower
section of page two. The form will print out on your printer.
- If you want to attach documents to the form, first select the
button "Submit by e-mail." A pre-addressed e-mail will open on
your computer. On the e-mail's menu bar select "Insert;" in
the dropdown window, select "File Attachment;" another window opens
- navigate to the file you want to attach and select it. If
you have other documents to attach, repeat this procedure until
complete.
- When you complete "attaching" support documents
OR if you have no
documents to attach, select the "Send" button on your e-mail.
The form will be automatically e-mailed to the Registrar.
